Field Care Supervisor - Bluebird Care Hounslow & Chiswick
Field Care Supervisor who will manage care assistants to provide safe, effective care and support that responds to each customer’s needs and preferences.

Roles and Responsibilities as a Field Care Supervisor Work well with customers • Prior to each service commencing, carry out with customers, and / or their chosen representatives, an assessment of what the customer needs and what they would like to achieve from their care and support, together with a risk assessment. • Create and agree a written individually tailored plan of care and support that respects the customer’s wishes and promotes their dignity and privacy. Agree appropriate risk control measures to reduce identified risks. Keep care and support plans up to date. • Provide the customer and where appropriate their representatives with information about the service so that they are clear about what to expect and how they can raise any concerns. • Keep all information about customers and their families secure and confidential. Work well with staff • Give care assistants appropriate information and guidance to effectively and safely provide care and support as agreed in the care and support plan. • Work with the Bluebird Care coordinator to appropriately match care assistants to customers and manage the flow of work. Attend the customer's first call to introduce the care assistant. Report changes to care schedules without delay to the coordinator, customer and care assistant. • Distribute, collect and monitor MAR charts. • Support with keeping staff files and records up to date • Provide advice, inductions, training, supervision and other support to the team of supervisors and/or care assistants • Take part in the out of hours emergency on-call rota as and when required. Effectively deal with emergency on-call issues, for example covering calls either directly or indirectly when care assistants are sick or absent. Record emergency calls and pass the information to appropriate parties. • Arrange and manage staff changeovers and briefings for live in care assignments in the absence of the care manager. Quality improvement • Carry out investigations relating to the quality of the service and implement improvement actions. • Monitor the performance of care assistants and carry out quality reviews at the customer’s home to make sure the service is being delivered in line with company procedures. • Apply Bluebird Care policies and procedures at all times and make sure accidents and incidents are recorded, reported and acted upon. • Make sure that care and support is provided in line with regulatory requirements and the Mental Capacity Act 2005 in England and Wales or Adults with Incapacity (Scotland) Act 2000 in Scotland. • Work effectively with customers, their families and representatives, other social and health care professionals and managers and staff from Bluebird Care to deliver high quality homecare services. Your Experience and Suitability for the Field Care Supervisor Role Experience of supervising staff in social or health care services Good understanding of the needs of people who require care and support at home and the principles of good care practice A relevant social care qualification or equivalent Experience of providing care and support to customers with all aspects of their daily living in a manner that respects their dignity, is non-judgmental and promotes their independence, choices and privacy Proven ability to carry our risk assessments and care and support planning centred on the individual needs and wishes of each customer You must have: Full and valid UK driving licence Access to a car Right to work in the UK Excellent level of English Because We Care About You We understand that every member of our team has their own family to care for and this is why our role offers the flexibility to work around your own home life. Care Coodinator
Bluebird Hounslow & Chiswick is recruiting a full-time Care Coordinator to join the team, to make sure customers receive excellent quality care and support to keep them safe and comfortable in their own homes. We're looking for an outgoing, supportive and empathetic individual with excellent organisational and communication skills. An individual who also enjoys empowering and supporting others. Key responsibility Make sure care experts are appropriately allocated to customers so that care is delivered on time, safely and in line with customers wishes (as agreed in their care and support plan). Duties • Liaise with the registered manager to make sure sufficient care experts with the right skills mix are recruited to meet the needs of the business. Arrange cover for care assistant sickness, absenteeism or holidays. • Accept, allocate and process new referrals for care and support promptly. • Process changes to customers care and support. • Make sure there is sufficient cover to deliver care and support on time. • Monitor the allocation of care experts to maximise efficiency whilst supporting them to maintain an appropriate work/ life balance. • Use IT systems to allocate care experts to provide care and support to customers. Work with Bluebird Care Field Care Supervisors to appropriately match care experts to customers taking account of: o Care experts skills, experience and time to safely deliver the care and support plan o The customer’s preferences and care needs o Travel arrangements, routes and working patterns to make efficient use of care experts time whilst meeting the needs of customers o Changes in care packages o Priorities when there are unexpected emergencies. • Ensure all rotas are completed at least 2 weeks in advance • Distribute staff rotas weekly. • Take part in the out of hours emergency on-call rota. This will only be required after the post holder has received full training and has been assessed as competent to provide appropriate guidance and advice in emergency situations. • Work with the office team to maintain up to date electronic and hand written records. Use systems to record and monitor mileage and travel distances. Make sure that accidents and incidents are recorded, reported and acted upon. • Keep all information about customers and their families secure and confidential. • Carry out general office duties. Prepare reports e.g. update folders, training records etc. of care experts as required by the registered manager. • Talk to customers and their chosen representatives about their care and support taking account of different communication needs and levels of understanding. • Work with the Bluebird Care team and other health and social care professionals to deliver high quality homecare services and to make improvements where necessary. • Carry out training of new and existing care experts if trained to do so. • Work flexibly as part of the team and apply Bluebird Care policies and procedures at all times. Skills and abilities: Knowledge of domiciliary care provision. Able to work as part of a team as well as under own initiative. Highly organised and able to prioritise.
